To officially reserve your slot, the next step is to complete your Enrollment Fee. Please fill out the Enrollment Form linked below. This form will let us confirm your payment details and move you forward in the process.

  • Option 1:

    Pay the standard Enrollment Fee – ₱2,500 to secure your slot

  • Option 2 (Optional): Complete your One-Time Full Payment - ₱31,500 upfront.

Once your payment is verified, you’ll be guided to submit your enrollment requirements and assigned to a Lifeline branch near you.
